To obtain a USSF D License Waiver, please register and pay the $25.00 waiver fee.
Currently in Utah Youth Soccer Association, a D License is required to be the HEAD coach on a U14- U19 Premier League team. (may be required for all Assistant Coaches in 2020)
**The D License waiver is good for 12 months, starting the date it is entered into your affinity coaching account.
All 3 steps must be completed for your waiver to be applied to your coaching account:
- Register and pay fee
- Complete the online Introduction to Grassroots course (in the Digital Coaching Center)* this should have been finished with a Grassroots license
- Email the completed waiver form to jbreckenridge@utahyouthsoccer.net
