To obtain a Grassroots or D Waiver, please register and pay the $25.00 waiver fee. You will need to complete one Grassroots online course through the Digital Coaching Center (DCC) 4v4, 7v7, 9v9 or 11v11
After you have registered, paid the waiver fee and taken the online Grassroots course; please send in the completed waiver form and Grassroots certificate via email to Holly Gundred: hgundred@utahyouthsoccer.net for approval from the TD, Eric Landon.
